Medical interventions against mesothelioma have limited effectiveness, in part, because they take place at too large a scale: Surgical implements and beams of radiation are many thousands of times larger than microscopic cancer cells, meaning a surgeon cannot remove every single diseased cell.  But the same issue does not apply to the body’s natural defense system, the immune system.  Within your bloodstream, microscopic immune cells attack pathogens and germs every day, fighting off colds and flus and ensuring that every bacteria cell you breath in does not multiply uncontrollably.
Researchers have reasoned that if we could harness the power of the immune system to fight mesothelioma, it could be cured: Immune cells could hunt down and target every cancer cell in the body. Although no immunotherapy drugs are currently FDA-approved, studies are showing the benefits of this approach, suggesting that these treatments could someday become the new standard of care.

Typically, it is difficult for the immune system to recognize cancer cells because they start out as the body’s own cells, which the immune system is designed not to attack. Foreign pathogens have proteins on their surface that the system recognizes as foreign and attacks. As normal cells develop into cancer cells, they too sometimes develop proteins that the body recognizes as foreign and attacks.  But cancer cells are mutating all the time, so they can evolve to evade this line of defense. There are various immune therapies designed to give the immune system a competitive edge.
The more basic kind of treatment are general immune system boosters like interluekinn-2, which helps immune cells divide faster. These are useful only if the system is attacking the cancer, but at too low a rate. Another type of drug is immune checkpoint inhibitors, which target the proteins that serve as “checkpoints” to stop the system from attacking the body’s own cells.  Some treatments work by trying to introduce large numbers of antibodies that bind to the surface of cancer cells.  Once an antibody is attached to a cell, it directs other parts of the immune system to attack it.
One class of drug that has shown progress is cancer vaccines.  These basically work by getting the immune system to respond to specific proteins carried on the surface of cancer cells.  Finally, experimental drugs work by attacking and destroying proteins that cancer cells use to disguise themselves as normal cells.

All of these drugs are currently experimental, but some could be close to going on the market. One drug that has shown significant results is avelumab, which aims to increase t-cell response.  It has been extensively tested and appears to be effective for people with late-stage mesothelioma.
