All forms of mesothelioma affect the same type of tissue: the flexible layer of cells surrounding bodily organs known as the mesothelium. However, depending on where in the body it takes place, the disease has different symptoms, prognoses and treatment plans.
Peritoneal mesothelioma is the second-most common form of the disease; it accounts for 10 to 20 percent of cases, and is thus a relatively rare version of this already rare form of cancer. Fewer than 500 new cases are diagnosed each year. This disease is contracted when a person breathes in tiny particles of asbestos; they are swallowed, or enter the lungs and make their way through the lymph system to the mesothelium, where they end up becoming embedded.  Over the years, they cause damage to cells, killing them, creating inflammation and making cells release harmful proteins that create mutations in surviving cells. Eventually the mutations can cause a cell to begin multiplying uncontrollably, resulting in a tumor.

Tumors usually form near the stomach and intestines.  They press against organs in this area and also cause fluid buildup, further pressing against them.  Patients’ first symptoms often include abdominal pain, loss of appetite, nausea, bloating, unexplained weight loss and intestinal blockage.  The disease is sometimes mistaken for other abdominal diseases like irritable bowel syndrome.  Once identified, treatments include chemotherapy, radiation and surgery.
Surgery is typically an option in early-stage cases where the tumors have not spread extensively; doctors typically remove as much of the cancerous tissue as possible. One highly effective is heated intraperitoneal chemotherapy: Immediately after surgery, doctors pump a heated saline solution mix containing chemotherapy drugs through the patient’s abdomen, with the goal of killing any surviving cancer cells that the surgery itself did not remove.  This form of treatment is more effective than traditional chemotherapy and increases survival time, but is typically only an option for people with less advanced versions of the disease. Studies have shown that people who have had this treatment have a 5-year survival rate of 25 percent.
Among patients with more advanced versions of the disease, treatment focuses on improving a patient’s quality of life by reducing pain and symptoms.  Radiation and chemotherapy alone cannot stop cancer from spreading, but they can reduce the effects of tumors by shrinking them in size.  Since fluid buildup is such a significant aspect of the disease, doctors may perform paracentesis, in which an incision is made to drain excess fluid that has built up.  No form of treatment for this disease is fully reliable, so clinical trials are taking place continually to identify promising new treatments.
