Pleural Mesothelioma

Mesothelioma is caused by breathing in carcinogenic substances, usually asbestos. Since the lungs are the entry point for the caner-causing fibers, it is most common for this form of cancer to affect them.
The mesothelium is a flexible, slippery layer that lines the body’s organs, including lungs, heart and chest cavity, while allowing them to shift as the body moves. Asbestos fibers affect this layer of tissue in particular because this is where the fibers end up after making their way through the tissue. (This is on contrast to lung cancer, in which smoke mainly ends up in contact with the inner tissues of the lungs.)

Once a person is exposed to asbestos, it takes a long time for disease to occur – 20 to 50 years. Eventually, in mesothelioma, a tumor starts to form when a cell that has acquired harmful mutations begins dividing uncontrollably.  This forms a tumor; the cancer cells can then spread to nearby areas in the lungs, lymph nodes and elsewhere in the abdomen. When tumors become large and widespread enough they can impede the function of the lungs.  They also irritate the tissues and cause fluid buildup, which further prevents the lungs from expanding and makes it hard to breathe. In the final stage of the disease, cancer spreads from the region around the lungs to other areas.
Symptoms include coughing, wheezing, shortness of breath and chest pain, as well as more general problems like fatigue and fever. Early warning signs are mild and can often be mistaken for cold symptoms, so at-risk people should always take these symptoms seriously.

Treatments for pleural mesothelioma include radiation and chemotherapy as well as surgery.  In radiation therapy, doctors map the location and shape of the tumor, and computer-guided machinery aims beams of high-energy radiation at it from multiple directions, with the goal of killing the cells and shrinking it.  Chemotherapy is also effective because it kills rapidly-dividing cells, including cancer cells as well as healthy cells like those in the hair follicle.

Surgery is usually the only option with a chance of eliminating the cancer and providing a cure.  Doctors consider curative surgery if the patient is in an early stage of the disease and otherwise healthy enough to tolerate it well.  Surgery can involve removing an entire lung and other surrounding tissues, just the lung, or just the lining of the lung.  Deciding which surgery to perform requires balancing the need to remove as much cancerous tissue as possible with the desire to preserve organ function. In late-stage patients, doctors may also perform surgeries to reduce symptoms and clear excess fluid from the lungs.

Once diagnosed, a patient with pleural mesothelioma typically lives about 12 to 18 months, but when the cancer is caught early and treated aggressively, they may survive 5 years or more.
